**YOUR CHILD HAS TO BE PRESENT**
Students must turn 5 years old ON OR BEFORE September 1, 2015.
Items needed:
•Proof of residence in the Dresden district – updated utility bill (gas, water, electric) or rent/lease contract in the parents name or an affidavit
•Certified copy of birth certificate
•Photo ID for parent or guardian
•Immunization Certificate-form 3231
•Hearing, Vision, and Dental-form 3300
•Social Security card (you may complete a waiver)
